We describe a woman with triple-negative breast cancer who develops significant hypoxemic respiratory insufficiency after radiation and immunotherapy. She is treated with both corticosteroids and infliximab with improvement. Once corticosteroids are tapered off, she has a relapsed pneumonitis requiring repeat hospitalization, increased in corticosteroid dose and infliximab. The case highlights management of relapsed pneumonitis and management strategies.
