This is a patient with metastatic renal cell carcinoma as well as pre-existing multiple autoimmune disorders including ulcerative colitis, ankylosing spondylitis developed acute quick onset of multi-organ failure (neuro, liver, kidney) after one dose of nivolumab leading to hospital admission. Despite thorough work up, and prompt treatment of high dose steroids, patient continued to deteriorate rapidly and deceased within the same hospitalization. However, his pre-existing autoimmune disorders e.g. ulcerative colitis remained dormant without any recurrent activity, even though the gastrointestinal toxicity is supposed to be more common compared to other organ toxicities.
