Our case was an elderly woman with small cell lung cancer who presented with grade 3 diarrhea and grade 2 colitis after 1 year of immune checkpoint inhibition with atezolizumab that was refractory to treatment with steroids and infliximab. Her symptoms improved after administration of vedolizumab and ustekinumab, but she was unable to take these medications due to a severe allergic reaction (sinusitis and arthralgias). Her condition stabilized after fecal microbiota transplant which enabled her to resume atezolizumab long term with favorable cancer outcome, however, had recurrent colitis symptom after 1 year, which benefited from repeat fecal transplant treatment.
