Malignant tumours of the maxillary sinus are rare, accounting for 1% of head and neck tumours. Treatment generally involves surgical resection with or without orbital exenteration. However, due to the tumour’s variability, combination therapy is often required when diagnosed in later stages. This typically involves surgery and postoperative radiotherapy, with the addition of chemotherapy, primarily platinum-based, for selected patients. Chemotherapy aids in organ preservation and reduces operative radicalism. Without neoadjuvant chemotherapy, radical surgery for advanced disease necessitates craniofacial resection, orbital exenteration, and total maxillectomy, leading to increased morbidity and cosmetic disfigurement. Common chemotherapeutic agents include platinum-based drugs like cisplatin, 5-fluorouracil, and taxanes, particularly Paclitaxel. The recommended protocols are further described in the chapter.
