Peritoneal Mesometrial Resection, Lymph-Collector-Guided First-Line Lymph Node Dissection and Extended Procedures

Robot-assisted cancer field surgery for the locoregional tumor control of endometrial carcinoma is described step-by-step. The standard procedure comprises peritoneal mesometrial resection (PMMR) and lymph collector-guided pelvic first-line lymph node dissection. Gonadomesenteric first-line lymph node dissection, second- and third-line lymph node dissection, and extended peritoneal mesometrial resections, indicated for regional and locally advanced diseases, are detailed as well. Recommendations for preoperative management, surgical equipment, patient positioning, intra- and postoperative complications, and their treatment supplement the presentation of the surgical techniques. Finally, the clinical potential of cancer field surgery for early-stage tubal and ovarian carcinoma is discussed.
