Widespread offering and availability of home-based cardiac rehabilitation (HBCR) programs are still very far from ideal. Even when HBCR programs are offered, lack of reimbursement reduces the rate of execution. Legislative action is mandated to relieve this roadblock. Training and availability of human resources also constitute a potential obstacle for implementation. Since the components of HBCR and center-based cardiac rehabilitation (CBCR) are very similar, HBCR can be implemented or taught by healthcare professionals who are already practicing CBCR. Information technology support to assure the necessary connections between patients and HCPs is also necessary. Institutional commitment to assure space as well as hardware and software resources is therefore crucial. Financial sustainability of a HBCR program can also be a significant barrier. Collaboration between a program and technology industry players could tender an alternative, building on needs such as long-distance monitoring of vital signs through wearable technology.
