Organizations have long sought to understand how to prevent workplace accidents to achieve expected levels of absenteeism reduction. A systemic perspective was adopted to comprehend the causes of accidents, considering the multiplicity of factors: human, workplace-related, organizational, and managerial. Workplace accidents arise from the combination of risks present and employees’ behavior. Human reliability is defined as the probability of a person not failing to perform tasks within a certain period, under appropriate environmental conditions, and with available resources. This study aimed to understand the determinants of human error underlying accidents. A data collection tool called “Contributions to the Transition to Industry 5.0” was developed to create a database for analyzing workplace accidents in 2022 related to human error, allowing the definition of strategies to enhance safety practices and reduce the incidence of accidents. Research questions were formulated to guide the development of an information collection tool among employees. Results revealed a low-risk perception associated with dangerous practices. Organizations should provide comprehensive training on safety procedures and protocols to minimize the risk of human error, emphasizing the importance of consistent adherence. Employee participation and company orientation towards the study objective were highlighted, clarifying doubts and enhancing understanding of human failure dimensions in accident investigations.
