Phototherapy of Uveal Melanoma

Phototherapy of uveal melanoma includes various forms of (a) photocoagulation, which raises the tissue temperature above 60 °C to cause immediate cell death; (b) transpupillary thermotherapy (TTT), which raises the tissue temperature to between 45 °C and 60 °C for about 60 s; and (c) photodynamic therapy (PDT), which uses light to activate a photosensitizer, thereby releasing toxic free radicals. Standard PDT uses verteporfin, which is infused intravenously. A novel method of PDT, developed by AURA Biosciences (Boston), currently under investigation, uses belzupacap sarotalocan (Bel-sar), a novel recombinant papillomavirus-like particle associated with a phthalocyanine photosensitizer, which is injected suprachoroidally. Photocoagulation has been superseded by TTT and PDT, which can be administered as primary treatment of the tumor or as an adjunct to radiotherapy or local resection, to treat any minimal residual disease or radiation-induced lipoid exudation, macular edema, or serous retinal detachment (toxic tumor syndrome).
