Laparoscopic Median Arcuate Ligament Release: Technical Aspects for a Safe and Effective Procedure

Laparoscopic median arcuate ligament (MAL) release is a surgical intervention for median arcuate ligament syndrome (MALS) that alleviates celiac trunk compression and addresses neurogenic elements by performing neurolysis of the celiac ganglion. While earlier studies showed no differences between the open and laparoscopic techniques, current research shows that the latter exhibits reduced intraoperative and postoperative complications, faster recovery times, lower hospital costs, decreased readmission rates, and improved immediate and long-term symptom relief. The potential limitations associated with the laparoscopic include challenges in controlling bleeding sources, the risk of incomplete compression release, and a higher likelihood of injury to major vascular landmarks. The critical nature of the procedure necessitates that it be performed by expertly trained surgeons in minimally invasive techniques and in medical centers equipped with readily available multidisciplinary teams consisting of general, vascular, and pediatric surgeons, gastroenterologists, and radiologists. This chapter details the steps for a safe and effective laparoscopic approach to MAL release, ranging from preoperative preparation to postoperative care.
