Inflammatory diseases are challenging in thoracic surgery. Tuberculosis and aspergilloma cause dense adhesions and complex anatomical distortion. Specialised surgical and anaesthetic strategies have to be developed for prevention of endobronchial spillage and safer dissection. We present our series of Robotic Lung Resections in patients with Aspergilloma, destroyed tuberculous lungs, and collapse lung secondary to inflammatory tumour. Robotic surgery provides a good tool for better visualisation, dissection and safe completion of complex surgeries. Both VATS and Robotics give good outcomes to the patient. Robotic assisted surgery gives 3D vision. This facilitates good dissection of the adhesions. The flexibility and manoeuvrability of the instruments allows safe dissection around the pulmonary hilum, pulmonary vessels and thoracic outlet. Ability to suture with the robot is second to none. It is intuitive and gives good results. Complex surgery like sleeve resection and sleeve lobectomy can be performed due to the availability of the robot.
