Coronavirus 2019 (COVID-19) has significantly changed the way we do healthcare. Today after the emergency phase is over, it is possible to do a retrospective study to evaluate how the healthcare system responded to the most critical phases in order to highlight strengths and weaknesses. This study aims to investigate the impact of COVID-19 in a general surgery department. The work is part of a research project started in 2021 involving several hospitals in southern Italy. In this case, data were collected on the activity and patients admitted to the department of general surgery at Evangelical Hospital “Betania” in Naples in 2019–2020. Using the year of discharge, the dataset was divided into two groups to assess the situation before and during the COVID-19 pandemic. The comparison was made by implementing statistical analysis and binary logistic regression. The results show that during the pandemic, there was a significant reduction in the number of cases treated. In addition, the appropriateness and complexity of treated cases increased statistically significantly (p-value = 0.002).
