The applications of intra-arterial drug delivery are constantly expanding, encompassing treatments for cerebral vasospasm, intracranial chemotherapy, and osmotic disruption of the blood–brain barrier. Ongoing research indicates that the safety and efficacy of drug delivery could be significantly enhanced by selectively infusing drugs into target tissues using superselective infusion techniques while temporarily halting blood flow. These procedures present technical challenges, necessitating the precise navigation of microcatheters, frequently with distal balloons, to control blood flow as needed. Advances in imaging, catheter technology, access techniques, and coagulation management are central to the complex radiological interventions performed today. This chapter reviews interventional radiology procedures to enhance the safety of arterial drug delivery.
