Duodenal injuries are uncommon and occur in 1–5% of patients who have abdominal injuries. Penetrating injury to the duodenum is more common compared to blunt injury. Remember that duodenal injuries rarely occur alone. It is important to look for other injuries. With any trauma, it is important to remember your Advanced Trauma Life Support (ATLS) protocol, stick to your primary and secondary surveys, and do a complete trauma workup. Do not forget one’s adjuncts, such as X-rays and Focused Abdominal Sonography for Trauma (FAST). If the patient is conscious and stable, do not forget to take a good succinct history—can use AMPLE mnemonic (Allergies, Medications, PMH/Pregnancy, Last Meal, Events related to the injury). Remember to note that staff should be assigned to place at least two large bore IVs while one is completing one’s trauma assessment.
