While youth with autism spectrum disorder require psychiatric hospitalization at a much higher rate than typically developing children, little attention has been paid to understanding the efficacy of psychiatric inpatient treatment or the translation of evidence-based practices to the inpatient setting. In this chapter, we describe a specialized psychiatric inpatient unit for children with ASD, including our own efforts to integrate evidence-based practices into the inpatient setting. We will begin by reviewing the risk factors for psychiatric hospitalization among children with ASD and the need for specialized inpatient units. Because specialized inpatient treatment programs are relatively rare, we will provide additional details about our experience of day-to-day operations, including the challenge of adapting practices to the inpatient setting. We pay special attention to recommendations derived from systematic reviews of evidence-based practices, note gaps in the research base, and offer recommendations based on our own clinical experience.
