Selected patients with local recurrence after initial surgery for rectal cancer are amenable to multimodality treatment including surgery with curative intent. The authors demonstrate feasibility of this approach which was associated with minimal perioperative mortality. While such an endeavor require an adequate infrastructure and cannot be simply replicated anywhere, these data do remind us that some individuals with locally recurrent rectal cancer can still achieve long-term cure.
