This landmark trial compared preoperative and postoperative chemoradiotherapy for the treatment of locally advanced rectal cancer. Oncologic outcomes and in particular long-term survival were overall comparable, but treatment toxicity was significantly decreased in the preoperative chemoradiotherapy group. Despite the recognized risk of overtreatment, these findings had a profound influence on the adoption of neoadjuvant rather than postoperative chemoradiotherapy as preferred standard of care for locally advanced rectal cancer around the world.
