A lymphoedema can be alleviated in severity by conservative therapy, but it cannot be cured. However, significant treatment success can be achieved through surgical procedures. Nevertheless, it is undoubtedly too early to speak of a paradigm shift in lymphoedema therapy. It is part of lymphological treatment to check the indication for surgical therapy. After VLNT: In a post-operative follow-up of 8.7 ± 4.2 months, the average reduction of the leg circumference after vascularised lymph node flap transplants (VLNT) was reported to be 64 ± 11.5% above the knee, 63.7 ± 34.3% below the knee and 67.3 ± 19.2% above the ankle (Cheng et al., Plast Reconstr Surg. 133:192e–198e, 2014). After LVA: In a recent publication, the circumference reduction of the extremities after a lymphovenous anastomosis operation (LVA) was between 20% and 43% (Tzou et al., J Surg Oncol. 121:91–99, 2020).
