The sleeve gastrectomy with transit bipartition (TBP), a variation of the biliopancreatic diversion with duodenal switch, was described in 2006 attaching the last 250 cm of ileum to the gastric antrum without division of the duodenum and an anastomosis of small bowel between 80 and 100 cm of the caecum. Although several publications have reported the results of this procedure, even the long-term ones, TBP has been seldom used as a conversion method after sleeve gastrectomy (SG). Only one publication, using a modified (loop) TBP after SG demonstrated additional weight loss 1 year after the conversion with figures comparable to single-stage TBP (% of excess weight loss >70% and % of total weight loss >40%). While nutritional consequences appeared limited, loose stools are frequent and the impact on gastroesophageal reflux is largely unknown. There are still very limited data on the conversion of SG to TBP which is motivated by weight loss issues with results similar to the single-stage operation in every respect.
