Conventional Mechanical Ventilation—I History, Mechanics, and Indications

Mechanical ventilation is a lifesaving intervention, evolved over centuries to its modern-day form. It is imperative to appreciate the historical developments to truly understand the evolution of mechanical ventilation to its modern forms, with safety and automation as an integral component of it, in today’s clinical practice. This chapter provides a brief narration of the history of mechanical ventilation highlighting significant developments. Understanding respiratory mechanics is paramount to learn the basics of mechanical ventilation and its various modalities, historic or current. A simpler version of physics and physiological principles are described to elucidate the mechanics of respiration during spontaneous and mechanical ventilation. Indications guiding the initiation of mechanical ventilation have been presented with a pathophysiological perspective to deepen the understanding of choice of a mode of mechanical ventilation.
