Comprehensive Fluid Therapy in Critical Care: Guidelines and Best Practices for Fluid Stewardship

Intravenous (IV) fluids are a cornerstone of critical care management but must be administered judiciously to avoid adverse outcomes like acid–base or electrolyte disturbances and fluid accumulation. This chapter outlines a comprehensive approach to IV fluid therapy, emphasizing the 10 D framework and the principles of fluid stewardship to optimize clinical outcomes. The 10 Ds focus on definitions, diagnosis, distribution, drugs, dose, duration, de-escalation, documentation, diligence, and discussion of fluids. It covers fluid resuscitation, maintenance, replacement, and de-resuscitation strategies and addresses fluid management in special populations where volume intake is restricted. Key guidelines for assessing volume status, fluid responsiveness, and fluid prescription appropriateness are detailed to promote patient safety and enhance the quality of care.
