Pharmaceutical therapy forms the backbone of treatment for the majority of modern diseases and, with the aging population, polypharmacy is common. As the use of these complex medication regimens grows, the risk of intentional and unintentional intoxications becomes increasingly challenging to identify and treat. This creates a unique role for the pharmacist in the early identification, triage, and treatment of these toxicities and serves as a liaison with local poison control. This chapter will review triage of intoxications through toxidromes and the frequent challenges associated with various drug screening methodologies. The chapter will delve deeper into the mechanism of toxicity, timeline, and treatment of common intoxicants such as acetaminophen, opioids, antidepressants, and others. Additional attention will be paid to less common intoxications that result in high acuity management and specific antidotes including, but not limited to, digoxin, toxic alcohols, and iron.
