Physical Activity, Balance, and Calf Muscle Pump Function in Chronic Venous Disease in Older Adults

The study aimed to investigate the relationship between venous hemodynamics of the Calf Muscle Pump (CMP), physical activity levels, and balance among individuals with Chronic Venous Disease (CVD). A cross-sectional, comparative, and correlational study was conducted, involving 21 adult participants with CVD (17 females, 4 males), aged 42–85 years. Data collection included a characterization form, assessment of physical activity using the Global Physical Activity Questionnaire (GPAQ), and evaluation of CMP hemodynamic functionality using photoplethysmography (BITalino). Variables measured included Volume, Refilling time, Venous Filling Index, Ejection Fraction, and Residual Volume Fraction during standing after lower limb elevation and heel raises. CVD severity was assessed using the Venous Clinical Severity Score, and balance was evaluated using the Tinetti Index and PLUX force platform. The relationship between CMP hemodynamics, physical activity levels, and balance was analyzed, along with the association between CVD severity and risk factors (age, Body Mass Index (BMI), sedentary lifestyle) and physical activity. Results indicated lower physical activity levels in adults with CVD (r = -0.55; p = 0.04), and better balance correlated with a higher venous filling index. However, no significant associations were found between CMP and balance variables with the risk factors. No other differences were observed among the three groups. In conclusion, individuals with CVD who exhibit low physical activity levels and high balance in this study demonstrated poorer venous hemodynamics of the CMP. Future studies should consider controlling for compression therapy.
