Advanced high grade soft tissue sarcoma of the extremities can be a challange to resect. In cases that are considered unresectable without amputation isolated limb perfusion with TNF-alpha and Melphalan (TM-ILP) can be a valuable aid in transforming these non-resectable tumors into a resectable state and thus effectively prevent amputation. This chapter describes treatment strategies, basic principles and an example case of a TM-ILP.
