Pulse Wave Velocity (PWV) and Endothelial Function Assessment: Principles and Pathophysiology

Pulse wave velocity (PWV) and endothelial function assessments are valuable noninvasive tools for evaluating cardiovascular risk. PWV measures the speed of blood pressure waves through arteries, indicating arterial stiffness, which increases with aging, hypertension, atherosclerosis, and diabetes. Common equipment includes Complior, SphygmoCor, and Vicorder. Endothelial function, assessed by flow-mediated dilation (FMD), reflects the ability of the endothelium to release nitric oxide (NO) and vasodilate in response to shear stress. FMD detects endothelial dysfunction, an early sign of atherosclerosis, and is influenced by cardiovascular risk factors like smoking, diabetes, and hyperlipidemia. Common equipment for endothelial function assessment includes brachial artery ultrasound, EndoPAT, and laser Doppler flowmetry. Clinically, elevated PWV predicts cardiovascular events and mortality, while reduced FMD identifies early atherosclerosis and aids in risk stratification and therapy guidance. Integrating these assessments into clinical practice enhances early detection and management of cardiovascular risk in men, enabling targeted preventive strategies and improved outcomes.
