Uterine Malformations

The uterus is the central organ of the pelvic cavity. Its size and morphology vary according to age and hormonal status. Its position and dimensions should be systematically included in every pelvic MRI report. MRI provides excellent visualization of uterine zonal anatomy. Typical physiological aspects, such as uterine contractions, must be recognized by the radiologist to avoid misinterpretation as pathological findings. Congenital uterine malformations are common and can be explained by embryology. Accurate diagnosis requires an optimized MRI protocol, ideally including vaginal opacification (if possible) and an oblique T2 sequence aligned with the endometrial cavity when present. The ESHRE classification is the reference for grading those malformations. MRI plays a crucial role in classification, as each malformation has specific implications for fertility and requires tailored management whenever possible.
