The dorsal deviations can be classified as linear, C-shaped, or S-shaped. In linear deviations, the use of splinting spreader grafts, disarticulation of the septal-ethmoid junction, or Cottle-type preservation techniques can be used to solve the problem. The situation with C-shaped deviations is more complex. There may be a need for traditional, modified, or splinting spreader grafts. Before applying the spreader grafts, scoring of the dorsal segment or removal of the angle of deflection may be performed. In revision rhinoplasty cases, the most popular technique is to use spreader grafts obtained from rib cartilages by oblique split technique.
