Shoulder reanimation can be achieved via tendon transfers for rotator cuff dysfunction and deltoid insufficiency. The authors preferred method for recreating the posterosuperior rotator cuff is by transferring the lower trapezius tendon, as it has a similar vector of pull to the external rotators. The anterosuperior rotator cuff can be recreated using the latissimus dorsi tendon, which is harvested in the axilla and rerouted to the insertional footprint of the subscapularis tendon. Axillary nerve dysfunction resulting in partial deltoid weakness can be treated with a pedicled pectoralis major muscle transfer, which is commonly performed in conjunction with a reverse shoulder arthroplasty. These tendon transfers can reliably and safely be performed with minimal patient morbidity.
