Intercostal nerves are typically spared in brachial plexus injury unless there is concomitant rib fracture. They are good donors for nerve transfers to biceps, triceps, deltoid, or a gracilis free muscle flap. They can be harvested through subperiosteal or intramuscular techniques. This chapter describes the subperiosteal technique.
