Robotic-assisted radical prostatectomy (RARP) has become the standard surgical approach for the surgical management of localized prostate cancer in the USA. While it offers numerous benefits, such as improved surgical precision and reduced morbidity, challenging cases in RARP require additional expertise and surgical finesse in several scenarios. These scenarios include patients with significant comorbidities, large prostate sizes, prior prostate surgeries, high-risk disease, and anatomical variations. Managing these cases demands a thorough understanding of the patient’s medical history, careful preoperative planning, and meticulous intraoperative techniques. This chapter aims to highlight the complexities and unique considerations, according to our experience, associated with challenging cases in RARP.
