The decision-making process for surgical indications in lower limb orthopaedic surgery, for children with cerebral palsy (CP), is complex. There is currently wide variation in surgical practice in this demanding field, which may contribute to unsatisfactory and unpredictable outcomes. To address the need for standardisation of surgical indications for lower limb surgery in children with CP, an expert international panel with more than 300 years of experience in 3-dimensional gait analysis and paediatric orthopaedic surgery was established. The most common procedures were identified, and consensus indications established using a Delphi process. Substantial agreement has been established and published for the most common lower limb surgical indications.
