ACGME describes family medicine physicians as being able to manage complex patient care through “coordinated interdisciplinary and interprofessional care.” In the family medicine clinic setting, interdisciplinary care may involve a variety of other medical professionals. Studies on interdisciplinary care have supported improved outcomes such as fewer emergency department visits, decreased hospitalizations, improved management of chronic disease, and decreased cost of care.
