This chapter explores the challenges and considerations in primary and secondary reconstruction, focusing on alternatives to the gastric pull-up conduit, such as colon and jejunal interposition. Colon interposition, while reliable, entails a higher morbidity compared to gastric pull-up, with reported leak and necrosis rates. Jejunal interposition poses challenges in obtaining a long, well-vascularized pedicled loop, although techniques like supercharging have shown promise.
