Survival after in-hospital cardiac arrest (CA) in children has improved significantly in the last decades. The use of extracorporeal membrane oxygenation (ECMO) as a resuscitation strategy following conventional CPR (ECPR) has also increased over time worldwide, likely contributing to the increased survival trend. In the 2015 update of the pediatric resuscitation guidelines, the American Heart Association supported ECMO consideration as part of the resuscitation strategy, particularly in cardiac patients, when expertise and equipment are available. In this chapter, we aim to review the ECPR definition, its objectives and indications, its epidemiology and outcomes, the main ECPR techniques including cannulation strategies, the most important peculiarities for specific ECMO populations, and details of post-ECPR care.
