This chapter describes the main findings of genetics and molecular biology of adrenocortical carcinoma (ACC) that could have clinical implications and should be considered by a clinician when approaching a new case of this rare disease. Therefore, the content of this chapter is deliberately concise and interested readers will find more exhaustive information in the literature cited. Five main macro-items will be discussed including germline DNA mutations, chromosome number alterations, somatic DNA/RNA alterations, epigenetic alterations and molecular biology of single molecules pathways relevant in ACC.
