Thanks to radiobiological and technical advances, radiotherapy (RT) plays a growing role in the management of adrenocortical carcinoma and pheochromocytoma. For adrenocortical carcinoma, adjuvant RT is indicated when a high risk of relapse is present. In the case of unresectable disease or local recurrence, the indications for RT are expanding as a result of the better understanding of tumor radiobiology and the availability of techniques capable of reducing toxicity while increasing their efficacy. In the context of palliation, RT may have a role in symptom relief. In selected stage IV patients with low disease burden, it is currently being investigated whether a greater impact on outcome can be achieved by the delivery of higher RT doses. Additionally, for pheochromocytoma RT shows a fairly good safety profile with no reports of hypertensive crises. Despite the scarce evidence guiding decision making, RT is suggested for palliation in metastatic settings.
