Multimodal approaches have been developed for patients deemed at high risk of recurrence and for those presenting with metastatic disease. These strategies integrate systemic therapy, surgery, and other local treatments in different clinical scenarios ranging from borderline resectable adrenocortical carcinoma (ACC) to oligometastatic and metastatic ACC. We discuss here the role of neoadjuvant systemic treatment, conversion therapy, the integration of cytoreductive surgery and hyperthermic intraperitoneal chemotherapy and the role of non-surgical local treatments in the management of primary ACC, its recurrence and metastases.
