Middle-aged weekend warriors are individuals generally older than the conventionally discussed athlete, and can live otherwise sedentary lifestyles that contribute to limited opportunities for regular exercise. The combination of high-energy physical activity with relative deconditioning, as well as older age and a potential history of competitive athletic play, can predispose them to a plethora of sports injuries. The knee is dynamically involved in a multitude of common recreational sports, such as soccer, football, basketball, baseball, softball, tennis, golf, and volleyball, and forms of casual exercise, such as swimming or climbing. A lack of proper conditioning and supplemental training in weekend warriors, particularly in comparison to elite athletes in these respective sports, can contribute to anterior cruciate ligament (ACL) or meniscus injuries. ACL reconstruction can improve function and enable return to sport in middle-aged weekend warriors, whereas activity modification and nonoperative treatment may be the preferred treatment in the patients that decide to escape from this lifestyle. Numerous treatment variables should be considered and discussed with the patient before surgery, such as timing, graft choice, and postoperative rehabilitation. Appropriately selected patients may benefit from ACL reconstruction and have similar complications and outcomes as younger athletes. Preserving as much functioning meniscal tissue as feasible while reducing pain and mechanical symptoms is the main objective of meniscal injury treatment.
