Posterior cruciate ligament (PCL) injuries in athletes occur less commonly than anterior cruciate ligament (ACL) injuries. As a consequence, there is less literature describing them, and their management has historically been an area of controversy. Research advances have improved the understanding of PCL anatomy and biomechanics, and have better elucidated the natural history, incidence, and presentation of PCL injuries. However, the optimal method of PCL surgical reconstruction remains a topic of ongoing debate. This chapter reviews current concepts in isolated PCL injuries and their management.
