Ulnar collateral ligament (UCL) injuries are becoming increasingly more common in the athletic population despite community outreach and preventative measures. Injuries are most common in the overhead throwing athlete but can occur in any sport. The ulnar collateral ligamentous is a complex anatomic structure comprising three main components: anterior, posterior, and transverse bands. The anterior band of the UCL is considered the most relevant in terms of athletic injuries and has the most isometry throughout elbow range of motion. A good clinical history, physical exam, and advanced imaging are all required for diagnosis and treatment decision making. UCL injuries can be managed both nonoperatively and operatively depending on patient and injury factors. There is extensive research examining the treatments for UCL injuries which have demonstrated favorable outcomes in the athletic population.
