Sport injuries have multidimensional consequences, but athletes will always suffer injuries. To minimize the negative consequence, practitioners face pressures to quickly return athletes to competition. Minor injuries present a unique challenge. Even though athletes can successfully return to competition before the tissue has fully healed, they will return with a greater likelihood of subsequent injury. Subsequent injuries may not always be associated with a previous injury. However, the authors of this chapter have commonly observed a so-called “magic bullet effect,” by which a cascade of subsequent injuries follow a minor injury, often culminating with a severe time-loss injury. Tissue healing timelines dictate the timeframe for which an athlete can return to play. During return-to-play, the athlete will reenter their sport until they are ready to return to competition. The goal of successful rehabilitation should not be to return to the same level of competition, but to return to performance—the same degree of competitive success. Supplementing the rehabilitation program with nutrition, strength and conditioning, and psychological support can optimize rehabilitation. A coordinated effort of the interdisciplinary support team, including shared return-to-play decision-making, can ensure the best outcome. Assessing overall physical fitness during return-to-play allows for refinement of the sport-specific strength and conditioning, and rehabilitation progressions to facilitate a successful return-to-competition. Reestablishing pre-injury physical fitness and sport-specific training capacity, while maintaining preventative therapeutic exercise, can support a successful return to performance while mitigating subsequent injury risk.
