The skin and gastrointestinal (GI) tract are complex organs that function as major interfaces between the host and the external environment. Both systems share key structural and functional characteristics, including large surface areas, epithelial barriers, rich vascularization, and extensive innervation. In addition, they harbor diverse microbiota that contribute to immune regulation and maintenance of homeostasis. Clinical associations between cutaneous and gastrointestinal diseases have long been recognized, and advances in immunology and microbiome research have provided further insight into these relationships. This chapter reviews the shared features of the skin and GI tract, with emphasis on microbiome–immune interactions and their role in disease pathophysiology, and discusses the clinical implications of this interrelationship.
