A thorough diagnostic work-up is mandatory to select patients affected by laryngeal cancer amenable to OPHL surgery. A precise awake and intraoperative endoscopic assessment is needed to check every single laryngeal subsite. Moreover, a high-quality imaging study with CT scan and/or MRI must be performed to check for deep laryngeal spaces and cartilages, which are critical to provide the best surgical indication to the patient.
