Anal Crohn’s disease may present with multiple perianal external skin openings. Management depends on disease severity. Asymptomatic perianal fistulas may not require surgical treatment. Surgical options include observation, drainage, fistulotomy, and advancement flaps. Fecal diversion and proctectomy may be needed in selected cases.
