Appendiceal adenocarcinoma is a rare gastrointestinal malignancy. Diagnosis typically follows the histopathologic evaluation of surgical specimens. When symptomatic, it can present in a manner similar to acute appendicitis or with a palpable mass. A high index of suspicion is needed in order not to miss the diagnosis, especially when initial biopsy does not reveal neoplasm.
