Full-mouth rehabilitation (FMR) has traditionally been guided by bone- or prosthesis-driven paradigms, which often fail to integrate the dentition, skeleton, and soft tissues into a cohesive functional unit, leading to compromised outcomes. Here, we introduce the Total Face Concept, a novel, maxillofacial-driven framework that treats the face as an integrated biomechanical and aesthetic entity. This concept is operationalized through two new synergistic frameworks: “Facial Rings & Slings” and “Orthoalveolar Form.” The “Facial Rings & Slings” model provides a functional map of the musculofascial network, elucidating the propagation of forces across distinct facial zones and explaining the far-reaching effects of localized interventions. Concurrently, the “Orthoalveolar Form” systematically classifies patients based on their maxillomandibular relationship and alveolar bone morphology, guiding foundational treatment strategy. Through a series of complex clinical cases—from severe dento-skeletal discrepancies to anodontia requiring extensive reconstruction with advanced grafting, zygomatic, and subperiosteal implants—we demonstrate the diagnostic and therapeutic power of this integrated approach. By synergistically applying these frameworks, clinicians can transcend isolated dental objectives to achieve predictable, stable, and harmonious rehabilitations that enhance the patient’s overall facial identity and well-being.
