This chapter critically examines the myths that exist about short-dental implants and provides evidence-based knowledge on their clinical performance. The newest systematic reviews and meta-analyses indicate that short implants (6–8 mm) perform similarly regarding their survival rate, marginal bone loss, and biomechanical stability to conventional-length implants but save on surgical complexity, treatment time, and postoperative complications. Implant macrodesign, bone quality (most notably D4 bone), primary stability, prosthetic factors, and risk factors specific to the patient are extensively addressed. Clinical treatment planning algorithms for the posterior mandible and maxilla are offered to maximize treatment planning. Additionally, risk factors like smoking and periodontitis history are examined for their contribution to short-implant success. Overall, this chapter promotes the evidence-based use of short implants as a minimally invasive, cost-effective, and biologically sound option for contemporary implant therapy.
