The tongue is a vital organ responsible for chewing, swallowing, tasting, cleaning teeth, vocalization, speaking, and breathing. Its musculoskeletal structure supports these functions, and impairment may require intervention. This chapter examines base of tongue surgery, macroglossia, and obstructive sleep apnea (OSA), covering classification, diagnosis, surgical interventions, complications, and strategies to reduce postoperative risks while optimizing airway function (Abd-El-Malek 1955; Thexton 1992; Simmonds et al. 2018; Morgan et al. 1996; Wolford and Cottrell 1996; Qaseem et al. 2013).
