Features of Goiter and Links with Iodine Deficiency: Indian Perspectives

Iodine deficiency remains a significant global public health concern, with critical implications for thyroid health, particularly in India. This chapter examines the complex link between iodine deficiency and goiter, drawing on insights from various studies and public health initiatives. Goiter, primarily caused by inadequate dietary iodine, presents with symptoms such as neck swelling, difficulty swallowing, hoarseness, and breathing problems. The chapter classifies goiters based on structural and functional features, highlighting iodine’s essential role in thyroid hormone synthesis. Efforts such as Universal Salt Iodization (USI) have played a crucial role in combating iodine deficiency. While India has made notable progress in implementing USI, challenges persist, particularly in rural and underserved regions. Socioeconomic and cultural factors, including vegetarian diets and limited access to iodine-fortified foods, significantly impact iodine consumption. Educational initiatives and targeted public health interventions are vital to addressing these issues. However, gaps in enforcing iodization laws and disparities in iodized salt distribution remain major obstacles. Improvements can be achieved by incorporating advanced technologies, strengthening monitoring mechanisms, and fostering public-private partnerships. Future approaches should prioritize robust monitoring systems, address environmental and dietary goitrogens, ensure access to iodized salt, and promote research and innovation in iodine supplementation. Policymakers must emphasize quality assurance to maintain adequate iodine levels among the population. India faces unique challenges, including regional disparities in iodine deficiency due to diverse dietary practices and environmental factors. High-burden states like Bihar, Uttar Pradesh, and parts of the Himalayan belt require tailored public health strategies. By adopting integrated and targeted approaches, India can significantly reduce iodine deficiency disorders and improve thyroid health across its diverse populations.
