Proximal Hamstring Tendinopathy

Proximal hamstring tendinopathy (PHT) is a chronic degenerative disease that presents as persistent buttock pain mainly during activities such as lunging, running, and sitting. This condition predominantly afflicts active women and men involved in sports like distance running, track and field, soccer, and rugby, albeit it can affect people who are not involved in any type of sport. Although the incidence of PHT is increasing, diagnosis is still missed or delayed because patients present with vague and idle symptoms, without a sudden specific injury. Treatment of PHT is mainly represented by conservative strategies focused on progressive load of the injured tendon in order to decrease pain, re-establish function, and prevent reinjury. Conservative treatment strategies include exercises, corticosteroid, platelet-rich-plasma (PRP) injection, and shockwave therapy. Surgical procedures for PHT are tenotomy, tendon and bursal debridement, and neurolysis/removal of adhesions around the sciatic nerve.
