Osseous metastases and multiple myeloma more commonly present in the pelvis, compared with primary bone tumours. Primary bone tumours in the pelvis, however, are more likely to be malignant than benign, with chondrosarcoma and Ewing sarcoma being the most common in the skeletally mature and immature populations, respectively, with the exclusion of haematological malignancies. The ilia are the most common pelvic sites in which bone tumours arise. Pelvic bone tumours are often large at the time of diagnosis, due to their deep anatomic location delaying presentation. This, along with the surgical challenges relating to tumours deep in the pelvis, are factors contributing to the worse prognosis of pelvic bone lesions, compared with their appendicular counterparts. Radiography remains a readily accessible and effective first-line imaging modality, with computed tomography and magnetic resonance imaging being valuable adjuncts, in the detailed assessment of pelvic bone tumours and for surgical planning.
